Student-Athlete Appearance Request Form
Please read the policies carefully and return the completed form to the Director of Athletics
Policies for Appearances:
- Completion of this form does not guarantee Cheyney will fulfill the request.
- Due to academic and time restrictions, student-athlete appearances will be made on a limited basis.
- Appearances will not be approved for student-athletes who are currently in-season (i.e., basketball student-athletes will not be available from October 15 till the end of the Division II National Championship)
- The Department of Athletics does not require the student-athletes to make appearances. Any appearance made by a student-athlete will be done voluntarily.
- The Department of Athletics does not facilitate appearances for former student-athletes.
- A signed copy of the NCAA Bylaw form must accompany this application. Contact the NCAA at NCAA.org (bylaw 12.5.1.1 and 12.5.1.1.2 and 13.16.1)
- The appearance of a student-athlete will in no way imply endorsement of the requesting organization by Cheyney Athletics.
